As part of the preparation process, Althea takes care of the Oregon Health Authority (OHA) forms through the system. Not only do we take care of it currently, but we plan to stay up-to-date on the latest regulations and ensure our system is compliant for Oregon regulated users.
When you see the OHA forms in Althea
In Althea, you are shown the OHA forms within a session's "preparation" screen when that session is associated with a licensed Service Center in Oregon. For any session that is either unassociated with a Service Center or associated with a center/clinic that isn't a licensed Service Center in Oregon, we don't add the OHA forms to the session.
Within preparation, there is a list of OHA forms. Below, you can see the session isn't yet connected to a Service Center, and so the OHA forms aren't included.
How to use the OHA Forms
By default, when you create a new session associated with an OR service center, we include the forms that are regularly required, but there are some additional forms that might be required depending on the session. You can add additional forms by clicking the Add Forms button. All the forms that are added to the session are included on the list of required items to be completed before the session moves to a status of "Ready."
Once you have your desired forms selected, click "Notify Participant." This opens a pop-up where you can input a few details in case you already have that information, such as a transportation plan. The info you put in here will get auto-populated on the forms that are sent to the client to save them time. If you don't have those details, you can simply click "Send."
Once you click "Send," the documents are automatically added to the client's portal and a link is sent to the client. The clients get to see the actual OHA PDF documents, but instead of filling out on the PDF, they fill out forms that populate the PDFs for a better user experience.
Once all documents are reviewed, filled out, and signed by the client, the documents are sent to the facilitator for review. At that point, the facilitator can meet with the client to review the completed information, ensure everything is appropriate and accurate, and then sign the documents themselves.
Once the client and the facilitator have both signed the documents, they are re-uploaded into Althea. Once all the required documents under preparation are uploaded into Althea, then the session moves from a status of "Preparing" to "Ready." The client is now ready for administration.
