We recently implemented custom forms as a part of Althea! What that means is you can send additional forms for clients to fill out and/or sign as part of preparation and integration. For everything on custom forms, check out the demo here.

To make custom forms even more useful, we have begun to create screeners that you can add in if you'd like!

The AUDIT-C Alcohol Use Disorder Test: is an alcohol screen that can help identify patients who are hazardous drinkers or who may have an active alcohol use disorder.

The Kessler psychological distress scale: is a widely used, 10-item questionnaire designed to screen for general psychological distress, particularly anxiety and depressive symptoms, in the general population. It's a brief and simple measure that helps identify individuals who may benefit from further assessment or treatment for mental health concerns.

The Self-Compassion Scale: is a 26-item self-report questionnaire designed to measure an individual's level of self-compassion. It assesses how people treat themselves during times of difficulty, failure, or suffering. The scale is based on three core components of self-compassion: self-kindness vs. self-judgment, common humanity vs. isolation, and mindfulness vs. over-identification.

The PTSD Checklist for DSM-5 (PCL-5): is a self-report questionnaire used to assess the presence and severity of symptoms of Post-Traumatic Stress Disorder (PTSD). It is based on the diagnostic criteria outlined in the DSM-5, the current diagnostic manual for mental disorders. The PCL-5 helps clinicians screen for PTSD, monitor symptom changes over time, and evaluate treatment outcomes.
