As of 2025, the OHA (Oregon's licensing authority) combined a few forms together into one form for state compliance: the psilocybin product and end of session form. In reality, this form has 3 parts that are best completed at three different points during a session. To accommodate a nice workflow, Althea's software pulls the form apart into its 3 sections so that you can complete them each individually, and then we collate all 3 parts back together at the end.
Here are those parts and how they work:
Estimated Dosages agreed upon during Preparation. You'll see that under the default Oregon forms, we list this form ("Psilocybin Product and End of Session Form"). During the preparation phase, we only send out the first piece of the form, which asks the facilitator and the client to agree upon the estimated dosing during a session. This includes both the initial dose and a booster dose. I believe they want this filled out so that clients can agree to dosing before they are under the influence. This helps set expectations for the client ahead of time.
Client Release Waiver. You'll find a link to this on the "administration" tab of a session. This is the 2nd part of the form, but since we really want the checkout process for a client to be seamless, we have the link here so that anyone can pull it open on a device, get the client and facilitator signatures, and send people on their way.
Psilocybin Product Information. You'll see a hyperlink to this form ("Psilocybin Product") at the bottom of "administration" tab of a session. This form should be completed after dosing has been administered and can be completed by the facilitator or a center administrator who has the details about the product and the waste.
As you can see, part 1 needs to be completed before the journey, and parts 2 & 3 should be completed on or after administration day. For that reason, we have separated the document into 3 parts, as I've shown.
Finally, once all parts are completed, you can always reference them by heading to the "preparation" tab of a session and clicking on the view link for the "Psilocybin Product and End of Session Form". You'll see that all 3 parts of the document have been put together for record-keeping there.
