Per 4 CCR 755-1 6.15(F), facilitators that are licensed in Colorado are required to provide a form titled Facilitator Required Disclosures, at each preparation, administration, and integration session. This form contains information that does not frequently change for different participants or sessions, like license number and contact information.
Because the information on this disclosures form doesn't usually change, we created a new workflow that allows facilitators to create a reusable template, so they don't have to fill out the same information every time they want to send this to a participant. To create your template, click on the icon in the upper-right corner of your screen, and then click Settings.
At the bottom of the page, you'll see the question "Are you a licensed facilitator in Colorado?" and if you select Yes, you'll be ale to create your Colorado Facilitator Disclosures form.
After you complete the template, you can review both the Facilitator Required Disclosures and the Fees, Termination, and Alternate Location Disclosures, and view the link to them that you can share with participants (they will also be available in the participant portal if added).
If you do not complete this template, then the Facilitator Required Disclosures and the Fees, Termination, and Alternate Location Disclosures will be blank in the participant portal.
Because the Facilitator Required Disclosures is required to be sent to the client during prep, administration, and integration sessions, a facilitator can add them to the set of forms that the participant sees in their portal. They will all show up in the Preparation Forms in the facilitator's Session view.
The Facilitator Required Disclosures is the preparation form, and will be included by default.
The Facilitator Required Disclosures (Administration) is for the administration session and can be added and sent at that time.
The Facilitator Required Disclosures (Integration) is for the integration session and can be added and sent at that time.
If it needs to be sent additional times, the facilitator can copy the link in the Settings page.
