Interpreter or Support Person Disclosure & Form (CO)

These two items are part of DORA’s Form 11 - Additional Observers, Support Persons, or Video Recording. They should be completed during the preparation phase and can be found in the Preparation section of the Session in Althea. Because they are only required as needed, they are not included by default in the list of required forms. If there is a need for an interpreter or support person, both the disclosure and the form should be added to the session, by clicking the ADD FORM button near the list of forms.

Interpreter or Support Person Disclosure

The disclosure is a static document with no fields to complete or signature required. It can be found here: https://app.withalthea.com/colorado-disclosures/support-person. As with all our read-only forms, our system provides the participant with a link to the form to review, and asks them to check a box when they reviewed the form. This will mark the form as “completed” in the session.

Interpreter or Support Person Form

This form must be signed by the participant AND the interpreter or support person. When this is added to the session and the client is notified, the facilitator can specify the email address of the interpreter or support person, which is how that person will receive the form to sign (they do not access Althea directly). If the facilitator does not know the email address, they can leave this blank for the client to fill out. The client will then sign the form and then the interpreter or support person will be notified via email and they can can also sign and submit the form.

Our system is set up for forms to be filled out by facilitators instead of support people, so there are some inconsistencies to highlight:

  • In the participant portal, after they have completed their portion, the status is “Pending facilitator signature” and it’s actually pending the interpreter/support person’s signature.

  • In the session, after the client has signed and it’s been sent to the interpreter or support person, it shows up as “Ready to sign” for the facilitator. Please ignore this, as the facilitator does not sign this form. When the interpreter or support person signs the form, it should show up as Complete, as expected.
