DORA Overview
Every form that is signed or attested to must include a date showing when it was completed or executed. Some forms also require the date of the administration session in addition to the signature date.
Forms completed during Preparation
(Required by regulation — 4 CCR 755-1)
These forms must be dated the day they are reviewed and signed, usually at the initial consultation or by the end of preparation.
Required dates:

According to the OHA rules, it seems like only the following pieces of info need the administration date on them:
Administration session record
Product/dose documentation (Psilocybin Product & End of Session Form)
SB 303 Client Data Form (one per administration)
The rest are supposed to be current and applicable and reviewed to ensure they work. Those should have a signed date but not necessarily the date of administration.
Practicums
How do I handle practicums where we may not know the date of administration until the day of?
Keep the practicums as one group session. From a workflow perspective, that seems to line up most with how training operations work.
Make a note for the dates of the practicum in the session notes.
For the form dates, use the first date of administration if needed for any preparation forms.
For the Psilocybin Product and End of Session Form, you can update the date when you fill out the Sign Client Release Waiver at the end of the administration. Ensure that the dates all align.
Multiple-Administration Retreats
For retreats that consist of multiple administration days, in Althea, you'll want to create a session for each administration date. But, since all paperwork isn't necessary for both administrations, you can simplify the process by focusing on doing paperwork for the 1st administration date.
Generally you may not need new forms for each administration as long as the plans for each administration are the same (transportation, safety, touch, etc.) Per Oregon, what you will need for each administration is:

SB 303 Client Data Form (one per administration)
So, for the 2nd session in Althea, you can simply use those 2 forms:

SB 303 Client Data Form (one per administration)
To further simplify the life of your client, for the SB303 Client Data form, you can simply download it from the 1st session and upload it to the 2nd session for them.
Then, ensure the Psilocybin Product and End of Session Form is completed for each participant, for each session.
